Information Release Form
This form is optional.
We often receive calls from parents or guardians looking to speak about a particular student’s assignment or bill. The Family Educational Rights and Privacy Act (FERPA) of 1974 is a federal law designed to protect the privacy of a student’s records. Other than in certain very limited circumstances, Residential Education and Housing (REH) will not release a student's information without written consent from the student. If a student would like to provide REH permission to speak with someone who is reaching out on behalf of the student regarding housing information, please complete an Information Release form. These can be completed at any time after this page on the Forms tab above.
Confidential Status established by the student through the OSU Registrar's Office overrides all release requests on file with other OSU offices, including REH. No information will be released pursuant to this consent while a student's registrar record is marked Confidential.
While you may have completed an information release with other OSU offices, this form is for OSU-Cascades Residential Education and Housing.
